Within the past 15 years there have been many recalls involving propane hoses and regulators in N. America.

I remember very large one involved the LP pigtail that attached to the cylinder. At the time we'd just bought a new 5er, both pigtails were in the recall. The replacements appeared exactly the same, but we never had any issues.
